Staff work on a hybrid schedule so the office is not always staffed during those hours but with prior notice, we can make arrangements for someone to be on-site.

To provide high quality, patient-centered primary health care in communities historically neglected due to racism and poverty, regardless of patients’ ability to pay.

About

The Institute for Family Health is a Federally Qualified Health Center (FQHC) and we operate community-based programs out of the FPC location. Those community-based programs include Dutchess County Healthy Families (DCHF) and the Perinatal and Infant Community Health Collaboratives (PICHC). Our free and voluntary home visiting services can provide support, information, knowledge, and resources to prenatal and parenting families in Dutchess County.

Perinatal and Infant Community Health Collaboratives (PICHC):

Short-term case management program aimed to connect individuals to community supportive services.

PICHC program seeks to prenatal individuals and families in achieving optimal health outcomes through connecting families to supportive services within their community. PICHC can assist participants with:

  • Health insurance enrollment and recertification.
  • Access to health care and finding medical providers.
  • Assistance with applying for public assistance.
  • Family Planning assistance and providing information about birth control options.
  • Pregnancy and parenting support.
  • Linkages to mental health/counseling, substance use treatment, and domestic violence services.
  • Connection to emergency and supportive services (housing, food, clothing, employment, education, health, and safety.
